Needs Assessment

The program would start with a thorough needs assessment of the senior and their living environment. This assessment would determine the necessary renovations and modifications needed to make the home more accommodating.

Funding Allocation

Once the needs assessment is complete, funding (up to a certain amount), would be allocated to cover the cost of renovations. This funding could come from a combination of government agencies, and possibly contributions from the seniors family.

Partnerships

Partnering with healthcare providers like Fraser Health would be essential for identifying eligible seniors, conducting needs assessments, and coordinating care services. These partnerships would ensure that seniors receive the necessary medical care and support services while living at home.

Renovations and Modifications

Qualified contractors would carry out the approved renovations and modifications to the senior's home. These renovations could include installing wheelchair ramps, grab bars in bathrooms, widening doorways, and other accessibility improvements.

Government Collaboration

Collaboration with government agencies would be necessary to secure funding, navigate regulatory requirements, and ensure the program aligns with existing healthcare policies and initiatives.

Monitoring and Evaluation

Monitoring and evaluation would be essential to assess the effectiveness of the program, identify areas for improvement, and ensure that funds are being used efficiently and effectively.
